Adoption Form

By completing this form, you are agreeing that your students will be logging into EHR Go with active, paid student subscriptions within six months of today’s date. If you do not start using EHR Go with students within six months, we retain the right to deactivate all faculty accounts and any purchased student subscriptions will be forfeited. Six months of no usage or no communication with your EHR Go representative will also result in deactivation.

If your course is only offered in the Spring or Fall, please let your EHR Go representative know to ensure all faculty accounts are kept active and any purchased student subscriptions will remain available for future use. Otherwise, six months of no student usage or communication with your EHR Go representative will result in the deactivation of your program and forfeiture of all purchased student subscriptions. If your account is deactivated and you wish to adopt again in the future, there will be a $250 reactivation fee to ensure your commitment and to cover the cost of the time and internal resources required to set up and support a program that adopts.
